These updated guidelines emphasize compliance, efficient claim processing, and optimal medical care to protect employers and employees.

This document highlights the critical reasons for promptly reporting workplace injuries or illnesses. It explains the benefits of early reporting, including reduced claim costs, compliance with state laws, and ensuring employees receive timely care. It also addresses common employer concerns about the impact on experience modification rates (EMR).

This guide outlines the immediate steps required after a workplace injury. It includes essential details for completing the Employer’s Basic Report of Injury form, such as employee and accident information, and instructions for submission to ensure timely reporting and effective claims processing.

4. Employer's Basic Report of Injury Form
This form must be completed and submitted to Accident Fund’s Express Claims Service and ChampionPEO’s HR Department within 24 hours of a workplace injury.

Employers, HR professionals, managers, authorized personnel, or incident witnesses covered under ChampionPEO’s Master Workers' Compensation Policy should reference these materials for guidance.


How to Use These Documents:

Steps to Submit a Claim:

  1. When an Injury Occurs:

    • During Normal Business Hours (8 AM - 5 PM, Monday-Friday):
      Call Accident Fund's Express Claims Service at 866-221-9640.
    • After Hours:
      Email the completed Employer’s Basic Report of Injury form to ReportClaim@AssignedRiskSolutions.com or fax it to 844-778-1069.
  2. Submit to ChampionPEO:

    After notifying Accident Fund, send the completed form to your ChampionPEO Account Manager or email it to HR@ChampionPEO.com.


  3. Additional OSHA Requirements:

    Employers must report fatalities within 8 hours and hospitalizations, amputations, or eye losses within 24 hours.
