The PTO Request and Authorization Form is a convenient resource available on our client-facing support hub for employees of our clients. It facilitates the request and approval process for paid time off (PTO) when employees cannot use their personnel portal via Champion Access.

Uses:

  • Alternative Request Method: This form allows employees to request PTO through an alternative method, ensuring flexibility in the request process.
  • Manager Approval: After filling out the first page, employees must send the form to their manager or immediate supervisor for approval, streamlining the approval workflow.
  • Submission to ChampionPEO: Once approved, the form is sent to the respective account manager at ChampionPEO or our payroll department at payroll@championpeo.com for implementation.

How to Use:

  1. Access the Form: Click the provided link to access the PTO Request and Authorization Form.
  2. Employee Information: Fill in your personal information, including your name, employee ID, contact details, and department.
  3. PTO Details: Specify the details of your requested PTO, including the start and end dates, the number of days requested, and any additional notes or comments.
