  2. Identity Disclosure (Optional): While anonymous submissions are accepted, providing your identity can be helpful for HR to follow up and gather more information if needed.
  3. Confidential Submission: Submit the form electronically to hr@championpeo.com
  4. Anonymous Submission: Click on Submit a Request in the upper right-hand corner of the Support Hub and put "Anonymous" in the name field and "Anonymous@Anonymous.com" in the email field.
