Submission:
- Who to Submit To: This form should be completed and submitted to the designated supervisor or manager at the client's company immediately after addressing any immediate safety concerns. It should be forwarded to ChampionPEO's HR department upon supervisor review for any necessary follow-up.
- Where to Submit: Forms can be submitted electronically via email to hr@championpeo.com or uploaded directly from the Support Hub under the 'Submit a Ticket' link at the upper right-hand of the Hub.
