Personnel File Review Request Form
Summary: This form allows authorized requestors, current or former employees of ChampionPEO, to request a review or a copy of their personnel files or specific documents contained within. Options include obtaining a digital copy, hard copy, or both. Certain charges apply to authorized requestors and current or former employees requesting copies. Requests must adhere to specific identification guidelines, and responses are generally provided within 30 calendar days.
